My shortcode refactor l

Fixed. My shortcode refactor left two render() methods in class-documentation-shortcode.php (lines 50 and 103) — the old instance-based one plus the new static one — causing the fatal on every request. Removed the stale duplicate; the single render() now delegates to render_shortcode() with no hook re-registration.

Per your rule, bumped to 1.3.1 (header + CWF_VERSION + readme Stable tag) and added matching entries to readme Changelog and Help page history.
